Your comprehensive evaluation includes:

• A 60–90-minute comprehensive visit

• Lab testing when appropriate

• An additional 30-minute lab review

• A personalized care plan

• 30 days of follow-up care

After your initial 30 days, you can choose a membership or individual follow-up visits based on your goals and the level of support you prefer.

Medication Management & Continued Care

Ongoing medications and prescription refills require follow-up visits at clinically appropriate intervals based on your medication, treatment plan, and individual needs. Required appointments and monitoring must be completed for continued prescribing.

This is a cash-pay practice. Office visits are not billed to insurance. Lab work, medications, supplements, and outside services are not included in the listed fees and may be billed to insurance separately when eligible.

A little extra support as you get started: When you enroll in a membership following your comprehensive visit, your first month of membership is included at no additional cost. Membership billing begins 30 days after your comprehensive visit.
